Clumped sperm, sometimes called gel-like semen, is a condition where semen becomes thick, sticky, or forms small clumps instead of being a normal fluid. This can affect sperm motility and reduce the chances of conception.

Causes of Clumped Sperm
Prolonged abstinence: Not ejaculating for a long time can make semen thicker and clump together.
Male reproductive infections: Conditions such as prostatitis, seminal vesiculitis, or urethritis can alter semen consistency.
Semen hyperviscosity: Excessively thick semen makes sperm movement difficult.
Lifestyle factors: Dehydration, smoking, poor sleep, or chronic stress.
Hormonal disorders: Low testosterone or other endocrine problems may affect semen quality.
Why Clumped Sperm is a Problem for IVF/ICSI
When semen clumps or is highly viscous, it can:
Reduce sperm motility: Sperm may get trapped in the thick semen.
Make sperm selection difficult: Harder to isolate high-quality sperm for IVF/ICSI.
Indicate inflammation or infection: This can negatively impact sperm health.
Can IVF/ICSI Still Be Performed with Clumped Sperm?
Yes, IVF/ICSI can still be performed using special sperm selection techniques:
MACS (Magnetic-Activated Cell Sorting): Uses magnetic beads to select live sperm with intact DNA.
PICSI: Selects sperm that bind to hyaluronic acid (HA), a substance surrounding the egg. Sperm that bind to HA are usually mature, healthy, and have good DNA integrity.
