Physical Examination
While many people already undergo routine annual check-ups, did you know that a general physical exam is not the same as a pre-IVF/ICSI screening? Let’s take a look at how they differ.
1. Ultrasound Scan
You might be familiar with ultrasounds for routine pelvic exams or monitoring pregnancy. However, an ultrasound before starting IVF / ICSI is specifically used to check for abnormalities in the ovaries and uterus. This includes screening for ovarian cysts or uterine fibroids—checking their presence, size, and whether their location poses any risks. In some cases, these conditions may need to be treated before the IVF / ICSI process can begin.

2. Hormone Tests
- Estradiol (Estrogen): The primary female sex hormone, which must be tested on day 2 of the menstrual cycle.
- FSH (Follicle-Stimulating Hormone) & LH (Luteinizing Hormone): Checked to evaluate ovarian response and help specialists calculate the appropriate medication dosage for ovarian stimulation.
- Prolactin: Since excessively high prolactin levels can directly interfere with the ovarian stimulation process, testing is required beforehand.
- Thyroid Hormones: These hormones regulate cellular metabolism throughout the body. Much like prolactin, thyroid levels that are too high or too low can negatively impact ovarian stimulation.
- AMH (Anti-Müllerian Hormone): Used to assess ovarian reserve (the remaining egg supply). For instance, a healthy, optimal AMH level typically indicates a good quantity and quality of retrievable eggs.
3. Infectious Disease Screening
Screenings include tests for HIV, Syphilis (VDRL), and Hepatitis B (HBsAg). Infectious diseases can negatively impact sperm count, motility, and quality, which in turn lowers embryo quality.
Long-term Impact: Frequent or chronic infections can lead to blockages in the sperm ducts (vas deferens), resulting in a complete absence of sperm in the ejaculate. This directly complicates sperm collection for IVF. Instead of a standard sample collection, advanced procedures like surgical sperm retrieval (e.g., TESE/PESA) may be required to extract sperm directly from the testes.
